Job Summary:

Provide comprehensive physical therapy services to patients in a skilled nursing facility setting. Focus on improving mobility, strength, and functional independence through tailored treatment plans.

Responsibilities:

• 
  Evaluate patients physical conditions and develop individualized therapy plans.

  Implement therapeutic interventions to improve mobility, strength, and flexibility.
  
  
• 
  Monitor patient progress and adjust treatments accordingly.
  
  
• 
  Educate patients and caregivers on exercises and activities to promote recovery.
  
  
• 
  Document all treatments, assessments, and patient responses accurately.
  
  
• 
  Collaborate with healthcare team members to optimize patient care.
  
  

Qualifications:

• 
  Hold a valid license or credential as a Physical Therapist.
  
  
• 
  Demonstrate clinical expertise in rehabilitative therapies, particularly in SNF environments.

  Possess proficiency in patient assessment, treatment planning, and progress documentation.
  
  
• 
  Exhibit strong communication and interpersonal skills.
