Case Manager

The Case Manager will be responsible for overseeing patient care and ensuring that necessary services are provided. They will coordinate with healthcare providers to facilitate effective treatment plans. The role involves assessing patient needs and collaborating with a multidisciplinary team. The Case Manager will maintain accurate records and documentation of patient care activities. They are expected to advocate for patients and their families throughout the care process. Effective communication and organizational skills are essential for this position. The position guarantees 40 hours of work per week.
