Speech-Language Pathologist (SLP) – Home Health

Starlight Home Health is seeking a compassionate and dependable Speech-Language Pathologist (SLP) to provide therapy services to patients in their homes. This is a great opportunity for an SLP who enjoys working independently while making a meaningful difference in patients’ safety, communication, and quality of life.

Responsibilities

• Complete speech therapy evaluations and reassessments

• Develop individualized plans of care and treatment goals

• Provide treatment for speech, language, cognitive-communication, and swallowing disorders

• Provide dysphagia assessment, treatment, and education as appropriate

• Educate patients, families, and caregivers

• Communicate changes in condition and recommendations to providers and the interdisciplinary team

• Complete accurate and timely home health documentation

• Coordinate care with nursing, PT, OT, providers, and other members of the care team

Qualifications

• Current Colorado Speech-Language Pathologist license

• Graduate degree in Speech-Language Pathology from an accredited program

• Current CPR/BLS certification

• Valid driver's license, reliable transportation, and willingness to travel to patients' homes

• Strong communication and documentation skills

• Home health experience preferred, but not required
