We collaborate closely with interdisciplinary facility teams, emphasizing open communication, shared clinical goals, and strong partnerships with patients and their families.Position OverviewWe are seeking a compassionate and experienced Psychiatric Mental Health Nurse Practitioner (PMHNP) to join our team. The ideal candidate excels at building rapport, takes a person-centered approach to care, and communicates effectively across disciplines. You'll work in collaboration with a multidisciplinary team to deliver comprehensive psychiatric services in post-acute and long-term care settings.Key ResponsibilitiesConduct follow-up psychiatric visits after initial assessmentsPrescribe medications and review diagnostic results as neededParticipate in team meetings and family sessionsCollaborate with supervising psychiatrists and facility staff to implement treatment plansMaintain timely documentation and communication with care teamsProvide education to facility staff regarding psychiatric care and patient needsEvaluate effectiveness of prescribed medications and make adjustmentsEnsure compliance with CMS guidelines and facility policiesQualificationsActive, unencumbered Illinois NP licenseBoard-certified PMHNP (PMHNP-BC)Master's degree in Nursing from an accredited programExperience treating mental health conditions such as anxiety, depression, dementia, and grief in adult or geriatric populations preferredComfortable working in skilled nursing and long-term care environmentsStrong interpersonal, organizational, and communication skillsAbility to collaborate with patients, families, and care teams effectivelyCommitment to patient confidentiality and professional standardsAdditional RequirementsParticipation in regular supervision with a psychiatristAbility to assess and manage psychiatric and behavioral health needsFamiliarity with psychotropic medication review and adjustment protocolsEducate facility staff on psychiatric issues and patient care strategiesIf you are a dedicated PMHNP ready to make a meaningful impact on the lives of our patients, we would love to hear from you!
