Licensed Practical Nurse (LPN)

As a member of the interdisciplinary team, the LPN assumes planning, responsibility, and accountability for resident care of a designated unit for one shift and in accordance with Federal and State regulations and company/facility policies, procedures and care plans.

ESSENTIAL FUNCTIONS

Lead and coordinate daily nursing operations by assigning staff, supervising resident care, ensuring appropriate staffing levels, and maintaining compliance with facility policies and resident rights. Provide clinical leadership by assessing residents, administering medications and treatments, implementing individualized care plans, coordinating with physicians and the interdisciplinary team, and responding to changes in resident condition. Supervise, mentor, and develop nursing staff through orientation, training, coaching, performance management, and ensuring compliance with infection control, OSHA, and regulatory standards. Maintain accurate clinical documentation and communication by completing required reports, care plans, incident documentation, shift reports, admissions/discharges, and communicating significant resident and operational updates. Promote quality care and regulatory compliance by monitoring care delivery, supporting quality improvement initiatives, preparing for surveys, overseeing operations in leadership’s absence, and ensuring a safe, clean, resident-centered environment.
