The Doe Fund has an annual operating budget of $88 million and a full-time staff of over 500, a significant portion of whom are graduates of our programs.

We are seeking a dedicated Case Manager/Mental Health Counselor for the Crystal Tower House Permanent Supportive Housing Program, which provides long‑term residential care and 24‑hour support services for homeless men and women living with HIV/AIDS. The program is designed to maximize residents’ independence while ensuring their physical and emotional security within a supportive community environment. Half of the residents will be referred by the New York City HIV/AIDS Services Administration (HASA), and the other half will be apartments for affordable to low-income households.

The Case Manager/Mental Health Counselor is responsible for delivering supportive case management services to HASA‑funded residents. The ideal candidate will demonstrate strong clinical skills, excellent communication abilities, and a deep understanding of the unique needs of individuals living with HIV/AIDS.

Key Responsibilities:

• Maintain a caseload and provide regular case management services to these individuals
• Maintain resident records in compliance with agency and funding standards
• Conduct and review assessments
• Develop and implement individual Service Plans and assist residents with achieving their goals geared toward self-sufficiency
• Monitor and ensure residents are linked to healthcare providers
• Provide counseling and crisis intervention as necessary
• Assist residents in securing and maintaining benefits
• Maintain a liaison with community services including hospitals, substance abuse treatment programs, HRA, and mental health programs
• Facilitate groups and activities that promote health and independent living skills development
• Monitor and assist residents with rent payments and budgeting
• Conduct regular unit inspections
• Educate residents about approaches to mental health
• Compile resources on mental health information and treatment strategies
• Attend ongoing training to enhance the services provided to the residents
• Assist with completion of HASA’s weekly and monthly reports as well as any other relevant reports
• Other duties as assigned

The Ideal Candidate:

• You have a Master’s Degree in Social Work or related field; a license in Clinical Social Work or Mental Health Counseling is preferred.
• You have at least one year of experience working with people who are homeless, living with HIV/AIDS, have substance abuse histories, or suffer from mental illness
• You have at least 5 years of experience in the mental health field
• You have knowledge of benefits in the New York City Metro area, HIV/AIDS, psychiatric diagnoses, substance abuse, and mental health issues
• You have solid computer skills – internet, Microsoft Office, and charting database
• You have great organizational, writing, and communication skills
