Care Manager - Passport/Assisted Living Waiver

About Us Western Reserve Area Agency on Aging (WRAAA) is a private non‑profit corporation organized by the State of Ohio to coordinate and administer federal and state aging programs in Cuyahoga, Geauga, Lake, Lorain and Medina Counties. It works with local service providers and the Ohio Department of Aging to provide home and community services and supports so that Ohioans can age successfully in their own homes and communities. Position Overview The Passport Care Manager is responsible for assessing, arranging, coordinating, monitoring, evaluating, educating, and advocating for a package of multiple services to meet the consumer’s needs in the community. Job Duties Assist Program Supervisor with clinical care management practices. Work with Program Supervisor on training and team development. Preceptor new staff; primary train-the-trainer with computerized program platforms. Manage a small caseload as needed. Conduct phone calls and in‑person contacts with clients to monitor service delivery. Respond to urgent/emergent needs for a case management visit. Authorize changes in amount, scope, and duration of waiver services. Collaborate with the Department of Job and Family Services to establish Medicaid eligibility. Assist Waiver Case Assistants with Medicaid, transportation, and service planning issues. Work with internal and external entities to resolve member program assignment issues. Assist dual‑eligible members in transitioning to the Integrated Care Delivery System (MyCare Ohio) as appropriate. Evaluate assessment and care management data to determine ongoing eligibility, care needs, and service options. Monitor and evaluate individualized care plans, service quality, and eligibility. Provide education and support to Waiver Service Coordinators, members, and caregivers. Advocate on behalf of consumers and caregivers and assist in securing appropriate services and care settings. Assign new members to staff as needed. Liaison to managed care programs and the Ohio Department of Job and Family Services. Support the AAA mission, vision, goals, and management decisions. Use clinical judgment based on education and professional standards, working with other team leaders. Abide by professional code of ethics; report abuse, neglect, or exploitation as required by Ohio law. Assist with special projects and communicate with management to improve processes. Meet time frames for timely documentation and maintain confidentiality. Perform duties directly related to the successful operation of the Agency, exercising discretion and independent judgment. Propose and participate in quality improvement efforts. Perform additional duties as assigned by supervisor to meet agency goals. Qualifications Current licensure as a Social Worker by the State of Ohio Counselor and Social Work Board, or as a Registered Nurse with the Ohio Board of Nursing. At least five years of experience working with individuals with physical or behavioral disabilities in a home health care, medical social work, or behavioral health environment; supervisory experience preferred. Current Ohio Department of Aging certification. Excellent verbal and written interviewing, assessment, and documentation skills. Basic knowledge of chronic disabling illnesses, disease processes, and normal aging. Effective communication with disabled and elderly populations and the general public. Collaborative communication and relationship building skills. Ability to comprehend written material related to essential job functions and basic medical terminology. Knowledge of local resources for older adults and persons with disabilities. Analytical skills to evaluate and implement appropriate actions. Ability to detect odors related to client health and safety. Willingness and ability to travel across multiple counties and access private homes and facilities; must have a car available during working hours and be a licensed driver. Basic computer skills, including Microsoft Office, and ability to transport a laptop; must lift up to 20 pounds. Benefits Competitive salary.
