Key Responsibilities\n

  • Conduct comprehensive and ongoing assessments to identify and address the mental health needs of patients.

  • Maintain open communication with supervisors, care staff, family members, and other stakeholders regarding each client's progress towards treatment goals.

  • Develop and submit tentative schedules for appointments, home visits, and observations, facilitating effective monitoring of work advancements.

  • Coordinate and execute home visits to perform in-home assessments, chart reviews, and validate data through interviews.

  • Ensure the effective implementation of personalized treatment plans in alignment with best practices.
  • \n
  • Participate in interdisciplinary team meetings aimed at enhancing care strategies and outcomes.

  • Document monthly and quarterly progress through detailed notes to track client developments and treatment efficacy.

  • Organize training sessions for staff to ensure competency in program implementation and compliance with documentation standards.

  • Provide follow-up support through interviews and home visits, emphasizing the continuity of care for clients.

  • Maintain accurate records accessible to stakeholders, ensuring privacy and regulatory compliance.

Qualifications\n

  • A Master’s or Doctoral degree in Counseling, Psychology, or a related field is required.

Compensation and Benefits\n

The annual salary for this position ranges from $90,000 to $100,000, commensurate with experience and caseload responsibilities.
