Flagstone is seeking a Nursing Assistant / Home Health Aide (Assisted Living & Memory Care) to join our team!

Position Details:

• Full-time: 30–40 hours per week AM weekdays
• Includes every other weekend
• Rotating holiday schedule

As part of a master-planned community offering senior housing, retail/commercial businesses, and market rate apartments, Flagstone is centrally located in Eden Prairie providing easy access to retail shops and dining while still being close to nature with beautiful views of Purgatory Creek. This community includes independent living, assisted living, memory care, and long-term care apartment homes as well as supportive services to provide older adults choices to live well today and every day!

Responsibilities

The Nursing Assistant or Home Health Aide, (Resident Assistant) is responsible for the provision of high quality personal care and services to residents to support the Personally Designed Living choices of residents, while maintaining consistency with regulations and established best practices.

Qualifications

• To be considered for Assisted Living: caregiving experience helpful but not required.
• Demonstrated competencies in person-directed, elder/resident-directed practice, home building, critical thinking, teamwork, self leadership, and interpersonal effectiveness.
• Demonstrated compatibility with the PHS mission and operating philosophies.
• Demonstrated ability to read, write, speak and understand the English language to communicate with all customers.

Pay Range

The estimated starting pay range for this role is $21.00 - $22.90 per hour. This good faith estimate is based upon PHS’s typical starting pay range for this role. Actual pay range depends upon many factors including each individual employee’s qualifications and market conditions and is subject to change.
