We are seeking a dedicated and compassionate Licensed Practical Nurse to join our team at The Avenue at Lyndhurst, a post-hospital rehabilitation and skilled nursing care facility in Lyndhurst, Ohio. As a key member of our healthcare team, you will play a vital role in providing high-quality care to our residents and patients. With a focus on comfort, dignity, and healing, our state-of-the-art facility offers a unique and supportive environment for individuals transitioning from hospital to home.

Responsibilities:

• Provide skilled nursing care and support to residents and patients under the supervision of licensed nurses and physicians
• Administer medications, treatments, and therapies as prescribed by physicians and according to established protocols
• Monitor and maintain accurate records of patient progress, medication administration, and treatment plans
• Develop and implement individualized care plans to meet the unique needs of each resident or patient
• Participate in interdisciplinary team meetings to discuss patient care and develop effective treatment strategies

Qualifications:

• Must be a Licensed Practical Nurse (LPN) currently licensed in the State of employment
• Genuine interest in geriatric nursing and long-term care
• Ability to make sound judgments and decisions
• Physically fit, mentally and emotionally stable
• Familiarity with facility policies and procedures
• Strong communication and interpersonal skills
