Job Description: - Join our dynamic team as an Occupational Therapist in beautiful San Jose, CA! - We are welcoming new graduates eager to make their mark in home health! - Your assigned territory will include: - Santa Clara - Sunnyvale - Cupertino - Milpitas - Mountain View - Campbell - Los Altos - Saratoga - Los Gatos - San Jose - Choose from TWO Options: - **Guaranteed Hours:** - Enjoy stability with guaranteed hours! - Achieve productivity of at least 27 points per week (details below). - Cover 36 hours guaranteed. - Complete 30 points in the workweek. - Flexibility to travel outside your area or work weekends if necessary. - Note: Guarantee does not apply to first and last week of assignment, holiday weeks, or weeks with time off. - **Non-Guaranteed Hours:** - Embrace flexibility with the non-guaranteed option! - Weekend coverage may be required. - Full caseload may involve occasional travel outside the service area. - Ability to refuse patients. - Payment made on a per-visit basis, with visit types converting to points: - OASIS ReCertification, OASIS RoC, Routine, Eval, Discharge, and others = 1 point = 1.33 hours. - Candidate Requirements: - Current CA OT License - BLS Certification - Valid Driver's License - Auto insurance - Auto registration - Annual TB screening - Immunizations (MMR, Varicella, HepB, TDap - within 10 years, Flu - annual) - Drug screening and physical as required. - Important: - All time off requests must be disclosed at the time of submission. - Requested time off after acceptance will not be approved.
