HIV Care Case Manager - Integrated Health & Social Support

Vivent Health

Vivent Health is seeking a Case Manager to join our healthcare team at our Denver clinic. You will provide comprehensive and compassionate case management to clients, coordinating medical, social, and emotional care with clinicians and community resources. Responsibilities include assessments, care planning, eligibility work, advocacy, crisis intervention, and robust documentation.
