You will complete hospice admissions, assess patient needs, coordinate services, educate patients and families, and connect them with the resources and support they need throughout the end-of-life journey.The ideal candidate brings strong clinical judgment, excellent communication skills, compassionate patient advocacy, and a genuine passion for hospice and palliative care.Key ResponsibilitiesComplete hospice admissions accurately, efficiently, and compassionately.Perform comprehensive assessments and determine appropriate hospice services.Educate patients and families about hospice philosophy, services, and available resources.Coordinate care with patients, families, referral sources, healthcare providers, and community partners.Build and maintain positive professional relationships with referral sources and healthcare partners.Document patient assessments, interactions, and services accurately in the electronic medical record at the point of care.Meet organizational performance expectations, including maintaining a 75% admission conversion rate.Follow all applicable hospice regulations, organizational policies, and standards of care.Advocate for patients and families while helping ensure a smooth transition to hospice services.Required QualificationsGraduate of an accredited nursing program.Active, unrestricted Registered Nurse (RN) license in the State of Ohio.Current CPR certification.Strong computer skills and proficiency with electronic medical records.Valid driver's license, reliable transportation, and a safe driving record.State-required minimum automobile insurance coverage.Ability to travel throughout the service area and drive during daytime, nighttime, and adverse weather conditions.Ability to successfully complete a background check and 10-panel drug screening.Preferred QualificationsAssociate Degree in Nursing (ASN) or Bachelor of Science in Nursing (BSN).Two years of acute care nursing experience.Hospice and Palliative Nursing (CHPN) certification.CHPN certification is preferred and must be obtained within two years of employment.
