Licensed Practical Nurse (LPN) - Home Health Full-Time, Direct-Hire MLR is partnering with a respected home health provider to find a dedicated Licensed Practical Nurse (LPN) for a full-time, direct-hire position. This role is crucial for delivering compassionate, high-quality patient care directly in the comfort of patients' homes. Key Responsibilities: Direct Patient Care: Administer medications, monitor vital signs, and assist with daily living activities. Collaboration: Work with physicians and registered nurses to implement care plans, including wound care, injections, and medical procedures. Documentation: Accurately document patient information in electronic health records (EHR). Patient Education: Educate patients and families on care plans and medication management. Compliance: Ensure adherence to home health regulations and advocate for patient needs. Licensed Practical Nurse (LPN) - Home Health Qualifications: Licensure: Current and unrestricted LPN license in the state of South Carolina. Experience: Minimum of 2 years of practical nursing experience, preferably in home health or a clinical setting. Certifications: Basic Life Support (BLS) certification required. Skills: Strong clinical skills, proficiency in EHR systems, and excellent communication abilities.
