Job Requirements

• Minimum 2 years of experience as a Home Health RN or in home health nursing
• Experience with OASIS and HCHB (Homecare Homebase) required - strong home health documentation skills
• Current RN licensure in the state of Massachusetts
• Current CPR certification (BLS)
• Valid driver's license, vehicle insurance, and access to a dependable vehicle or public transportation

Responsibilities

• Deliver high-quality, patient-centered nursing care to adults in their homes, ensuring safety and comfort
• Perform comprehensive patient assessments and develop individualized care plans for home health patients
• Document patient progress and clinical findings using OASIS and HCHB for accurate home health records
• Coordinate care with interdisciplinary teams - therapists, physicians, social workers - to support post-acute care and ongoing home care
• Provide patient and family education on disease management, medications, wound care, and home safety
• Maintain compliance with regulatory, clinical, and agency standards for home health nursing
