SALARY: $140K - $162K, DOE

$10,000 signing bonus

DESCRIPTION

As a member of an interdisciplinary health care team, the Nurse Practitioner is responsible for providing complete clinical care for patients of all ages using a patient centered medical home model of care.

RESPONSIBILITIES

• Under the direction of the Medical Director and per written protocol provides care consistent with community standards and follows the rules of the various payors.
• Confers with primary care physicians or specialists regarding the care of patients as well as medical and diagnostic work performed.
• Participates in Quality Assurance Initiatives and provides input on the development of policies and procedures, which impact the health delivery service. 
• Participates in training relevant to the education of professional health personnel as well as any training required by the various funders. 
• Provides exemplary customer service.
• May function on a rotating basis as the provider on-call during after-hours.
• Maintains the security and privacy of all Protected Health Information. 
• Documents all information accurately and timely using an Electronic Medical Record System.
• Participates in all safety programs, which may include assignment to an emergency response team.

QUALIFICATIONS

• Graduate of an accredited Nurse Practitioner Program.
• Current license in the State of California as a Nurse Practitioner.
• Must possess current DEA license and CPR (BCLS/ACLS) certificate.
• Strong communication skills in English and the ability to speak Spanish is preferred.
• Relate well to people of various social-economic backgrounds
