Physical Therapist (PT) – Home Health

Schedule / Job Type

Contract | Full-Time | Days | Monday–Friday, 8:30 AM–5:00 PM | Onsite / Field-Based (Home Health)

Key Responsibilities

• Provide in-home physical therapy services to a caseload of 4–6 patients per day across a defined service area
• Perform patient admissions, ongoing treatment of existing patients, and recertification evaluations
• Treat a multidisciplinary adult patient population including elderly, post-CVA, post-surgical, orthopedic, and wound care cases
• Participate in multidisciplinary team meetings held on Mondays and Wednesdays to discuss current admissions
• Document patient care using Epic EMR with a remote client offshoot
• Begin shift directly from home; travel throughout the assigned service area (up to approximately 60 miles east and west, 60 miles north, and 40 miles south)

Required Qualifications

• Active Physical Therapist (PT) license in good standing
• Minimum of 1 year of post-graduate PT experience
• Prior home health experience required
• Orthopedic experience required
• Basic Life Support (BLS) certification through the American Heart Association (AHA)

Preferred Experience / Skills

• Post-graduate home health experience strongly preferred
• Experience with Medicare patient populations and rehab-focused care settings
• Familiarity with Epic EMR
