Outpatient Registered Nurse - RN - Dialysis

Purpose and Scope The professional registered nurse Outpatient RN CAP 2 may be an entry-level designation into the Clinical Advancement Program (CAP). This position is accountable for the provision and coordination of clinically competent care for an assigned group of patients, delegating tasks to RNs, LVN/LPNs, and Patient Care Technicians. As a member of the End Stage Kidney Disease (ESKD) health‑care team, the RN participates in decision‑making, teaching, leadership, and quality‑improvement activities that enhance patient outcomes and facility operations. Principal Duties and Responsibilities Perform ongoing, systematic collection and analysis of patient data for assigned patients and document in the medical record. Make adjustments or modifications to treatment plans as indicated and notify Team Leader, Charge Nurse, Supervisor or Physician when needed. Assess, collaborate and document patient/family learning needs to provide education. Direct and provide the daily provision of safe and effective hemodialysis therapy to assigned patients. Administer medications as prescribed; document justification and effectiveness. Initiate or assist with emergency response measures. Serve as a resource, leader, coach, mentor and role model for new and incumbent employees. Ensure correct laboratory procedures and reschedule missed or insufficient collections. Identify expected outcomes and update the nursing assessment and plan of care through collaboration with the Interdisciplinary Team. Ensure patient awareness of transplant and treatment modality options. Participate in education and quality‑improvement projects at the facility and area level. May serve as a preceptor to new employees and complete CAP requirements to maintain or advance. Perform all other duties as assigned by Supervisor. Physical Demands and Working Conditions Direct patient care that regularly involves heavy lifting, moving patients and assisting with ambulation. Frequent, prolonged standing and the ability to bend over are required. Movements of equipment up to 200 lbs and lifting solutions up to 30 lbs as high as 5 feet may be necessary. The work environment is a health‑care facility with air temperature control and moderate noise levels. Exposure to infectious diseases and materials may occur. The position may require travel to training sites or other facilities, with the same physical demands and working conditions applicable elsewhere. Supervision The RN has oversight of Patient Care Technicians, LVN/LPNs, and RNs as Team Leader or designated Nurse in Charge after successful completion of FKC education and training requirements and a minimum of 3 months' experience in chronic/acute hemodialysis as an RN. Education and Licensure Graduate of an accredited School of Nursing. Current, appropriate state licensure. Current or successful completion of CPR/BLS certification. Meet the practice requirements in the state of employment. Experience and Required Skills Entry-level RN with a minimum of 2 years of Nephrology Nursing experience. Ability to perform the essential functions of the job.
