Offer Based on Years of Experience What You Need to Know If you meet these qualifications, we would love to meet you: Must have minimum of a Bachelor of Social Work (BSW) degree from a school of social work accredited by the Council on Social Work Education, Master of Social Work (MSW) preferred. Successful completion of an approved undergraduate level (BSW) or graduate level (MSW) hospice internship Qualifications of state-specific requirements above minimum education standards At least one (1) year of experience in a health care setting Medicare/Medicaid/Insurance specialty preferred Responsibilities: As a Hospice Social Work Fellow, you will: Assist the core members of the pre-hospice/hospice team in understanding significant social, spiritual, and emotional factors related to the patient's health, to establish a plan of care which fosters the personal worth, spiritual well-being, and human dignity of each patient Participate as a member of the interdisciplinary team and in the development and review of the plan of care for assigned patients Prepare the patient to deal with the changes and the family to support the patient including education on Advanced Directives Utililize all available resources, such as family, hospice, and community agencies, to assist the patient and family to live better within the limitations of the illness Support the bereavement program at the site level Our Investment in You Caring for others starts with caring for you.
