Youth Transition Coach & Case Manager
Wayside Youth
Wayside Youth is seeking a dynamic Transitional Facilitator/Case Manager in Waltham, MA to support DCF young adults (18-22) in transitional housing. You will coordinate intake, meet weekly, and develop service plans with participants to promote stability and growth. The role requires 1+ year experience with adolescents/young adults and residential programs; a driver's license and reliable vehicle are required.
