To support patients with chronic eye diseases, the full-time Continuous Care Coach will assess, coordinate, and manage care needs remotely, engaging with up to 15 patients daily to provide encouragement and support. Key responsibilities Manage patient caseload and communicate frequently based on assessed needs Develop and implement personalized care plans in collaboration with patients, families, and healthcare providers Educate patients and families on self-management and facilitate access to necessary care and resources Required qualifications Active Certified Ophthalmic Assistant (COA) or Certified Ophthalmic Technician (COT/COMT) designation 2+ years of work experience in eye care preferred Proficient with Electronic Medical Record (EMR) systems, preferred Ability to work effectively in a remote environment with reliable internet Strong consultative skills to provide guidance and support to patients
