PRN Physical Therapist (PT)

Resolve Harmony is seeking a PRN Physical Therapist to join our rehabilitation team. Responsibilities Evaluate residents and develop individualized treatment plans Provide skilled physical therapy services in accordance with physician orders Assess mobility, strength, balance, and functional abilities Collaborate with physicians, nursing staff, and rehabilitation team members Maintain accurate and timely documentation Participate in care planning and discharge planning as needed Educate residents, families, and caregivers on treatment goals and home exercise programs Ensure compliance with regulatory and professional standards Qualifications Graduate of an accredited Physical Therapy program Current Physical Therapist license in the state of practice Skilled nursing, long-term care, rehabilitation, or post-acute experience preferred Strong clinical assessment and documentation skills Excellent communication and interpersonal abilities Ability to work independently and as part of a multidisciplinary team Competitive PRN rates Supportive therapy and clinical team Opportunity to make a positive impact on resident outcomes Qualified candidates are encouraged to apply for immediate consideration. #J-18808-Ljbffr
