Hospice NP (PRN) - Day Shift | Mileage Reimbursement

CommonSpirit Health at Home is seeking a Nurse Practitioner - Hospice (PRN) to perform face-to-face assessments and determine hospice eligibility. Role may include home, physician office, or inpatient hospice settings with a 8:00 AM–5:00 PM schedule and no after-hours on-call. You will educate patients and families, develop care plans, and coordinate with the care team, documenting promptly.
