Home Visitation Specialist

We are currently looking for a Full Time Home Visitation Specialist to join our Family Preservation Team team located at Ardmore working 8 a.m. to 5 p.m. M-F.

Summary:
Serves as an advocate for families with native children in the home who are at risk of removal or have been removed as a result of child abuse, neglect, domestic violence, substance abuse, and other violent or traumatic behaviors. Specialist will carry out client intakes, provide home visitation, in home parenting education, and resources, while supporting a case plan consisting of specific interventions and goals created by the family in an effort to stabilize the home, and create a safe and nurturing environment.

Responsibilities:
* Screens prospective clients to identify risk of removal, and conduct intake with clients if risk factor is identified
* Visits homes of families for assessment on a regular basis and accompany Tribal and/or State Child Welfare Services when families are under investigation
* Provides advocacy for the children in all manners of safety and stability
* Provides information and referral services to families as indicated
* Acts as a liaison between the professional resource and the client/family
* Prepares case service plan with the families input
* Provides follow-up and monitor clientele
* Prepares Title IV-E documentation and correspondence with the Title IV-E staff
* Completes specific tasks related to Personnel Activity Reports (PARs)
* Transports Chickasaw citizens and other First Americans to and from appointments, meetings, and activities as needed
* Develops and maintains documentation as appropriate - files, logs, records, reports, etc.
* Maintains spreadsheets and databases and inputs data entry
* Collects required documentation from participants
* Prepares monthly reports
* Completes all training and development requirements and participates in continuing education opportunities
* Works in a collaborative and supportive manner to support the productivity and safety of the work area, team members and customers of all types
- Maintains a clean, organized work area and ensures a safe environment

- Punctual and consistent in meeting start time, work schedule, attendance and other assigned work time metrics
- Adheres to Chickasaw Nation dress code standards
* Maintains knowledge of and follows all rules, regulations, policies and guidelines for assigned area
* May engage in public speaking to schools, community organizations, employee groups, citizen meetings, events, etc.
* Performs all other duties as assigned

Skills:
Essential:

Experience:

Nonessential:
* 3 years directly related experience

Education:
Essential:
* High School, GED or Equivalent

Nonessential:
* Bachelors Degree in directly related field

Credentials:
Essential:
* Standard Driver's License
* Pass general background check
