In-Home Health NP Contractor (Flexible Schedule)

Job Overview Signify Health, part of CVS Health, is hiring board‑certified nurse practitioner contractors to conduct In‑home Health Evaluations for Medicare Advantage, Medicaid, and certain commercial plan members. Key Responsibilities Conduct in‑home visits focused on the patient’s overall health and well‑being, identifying potential barriers to care. Complete medication and medical history reviews, and perform physical evaluations in the patient’s home. Use a pre‑populated, patient‑specific form for each evaluation. Do not prescribe medicine, order lab tests, or alter existing treatment regimens. Qualify for or enroll in a Medicaid license (preferred). State license required. Work short‑term or long‑term basis with scheduled in‑home visits. Receive malpractice coverage and availability incentives based on weekly availability. Operate as an independent contractor (1099 status) with a flexible schedule. Compensation & Schedule Earn up to $2,500 to $4,000 each week (average 6–9 visits daily, base rate $80 per completed assessment). Increase per‑visit rate up to 135% with the Market Multiplier. Receive additional pay‑per‑test compensation for visits that qualify for diagnostic or preventive tests. Shifts available Monday–Sunday; no nights or weekends required.
