Location: Glasgow Independent Schools Healthy Kids Clinic, Glasgow, KY 42141. Position Type: Full Time. Job Category: Health Care – LCSW / LPCC / LMFT. Role Description The LCSW/LPCC/LMFT provides comprehensive psychosocial assessments and behavioral health care. Services are designed to support and maximize the client’s ability to better direct their own health care experiences using assessment, intervention, education, and linkage to health and community resources. The provider respects individuality, independence, patient choice, and collaborates on policies and systems that respect cultural, religious, and ethnic differences among patients and families. Mental health services contribute to the medical team’s understanding of patient beliefs, behaviors, and barriers regarding the client’s illness. Responsibilities Use age‑appropriate assessment and intervention skills; apply a range of interventions based on patient/family needs. Assist members in developing cost‑effective, appropriate health care plans by optimizing personal resources and community programs. Promote multidisciplinary, patient‑centered care that considers all aspects of members’ personal, psychological, economic, and cultural needs. Maintain thorough knowledge of CFMC services/benefits, public/private community resources, and federal/state laws affecting health care planning. Complete required documentation of services and plans of care per policy. Participate in program development, orientation, and educational activities to enhance members’ capacity for health care decision‑making. Other duties as assigned. Qualifications 1 year of experience in mental health, hospital, medical outpatient clinic, FQHC, home health, or long‑term care setting. 3 to 5 years of related professional experience. Master’s Degree and required licenses, certifications, and registrations. Current unrestricted license in the state of practice required upon hire. Basic Life Support (BLS) for Health Care Providers. National Provider Identifier (NPI) required prior to employment start date. Key Skills 3 to 5 years of professional experience in a hospital or medical outpatient clinic, FQHC, Home Health, or long‑term care setting. Expertise with brief counseling interventions. Demonstrated ability to provide leadership. Advanced knowledge of local and national resources related to behavioral health care. Schedule Full‑time, 5 days per week, 100% clinic‑based, in‑person work required. Work schedules are set at the discretion of CFMC and may be adjusted at any time based on organizational operational needs. Compensation Negotiable based on experience.
