Responsibilities
- Conduct comprehensive PT evaluations for residents and develop individualized treatment plans
- Implement therapeutic interventions to improve mobility, strength, balance, and functional independence
- Perform gait training, therapeutic exercises, and manual therapy as appropriate
- Monitor patient progress, document outcomes, and adjust the plan of care in collaboration with the interdisciplinary team
- Educate residents and families on home exercise programs, safety, and fall prevention
- Ensure compliance with facility policies, documentation standards, and infection control practices
- Participate in discharge planning and coordination with care transitions as needed

Requirements
- Ability to work effectively in a skilled nursing setting with geriatric populations
- Willingness to commit to a 14-week travel assignment
- Availability for a daytime schedule (8:00 a.m. – 4:30 p.m.), Monday through Friday
- Strong communication and collaboration skills for interdisciplinary care
- Proficiency with standard PT documentation and electronic medical records
