Responsibilities
- Provide direct nursing care to residents in a long-term care setting, including monitoring, assessments, and routine clinical tasks
- Administer prescribed medications and document administration accurately; observe and report adverse reactions
- Assist residents with activities of daily living and support functional independence
- Monitor resident conditions, perform vital signs checks, and escalate changes to the licensed clinician as needed
- Collaborate with interdisciplinary team to implement care plans, ensure continuity of care, and maintain thorough, timely documentation

Requirements
- Active Licensed Practical Nurse (LPN) license
- Experience in long-term care or geriatric care settings preferred
- Completion of an approved practical nursing program
- Ability to work 3x12-hour day shifts (6:00 a.m. – 6:30 p.m.) in a travel assignment
- Strong communication and documentation skills; reliable team player
