ARMStaffing is looking for a Registered Nurse, Case Management (Day Shift) in San Leandro, California.

Shift will be Monday - Friday 8:30 AM - 5:00 PM.

Job Description

The Registered Nurse in Case Management coordinates the management of care to ensure optimal utilization of resources and compliance with external review agencies. This role includes conducting comprehensive assessments, formulating care plans, implementing those plans, and evaluating individual patient needs while ensuring patient preferences are met. The RN enhances patient management quality and satisfaction, promotes continuity of care, and manages costs through integration of case management, utilization review, and discharge planning functions.

Responsibilities

• Coordinate management of care and resource utilization.
• Conduct comprehensive assessments and care planning.
• Implement care plans and evaluate patient outcomes.
• Promote patient preferences and advocate for their needs.
• Oversee discharge planning and address holistic patient needs.
• Collaborate with interdisciplinary teams to ensure effective service delivery.

Required Experience

• Minimum 2 years of recent RN case management experience.
• Experience in a Long-Term Acute Care (LTAC) or Short-Term Acute Care Hospital (STACH) setting.
• Experience with MCG guidelines and discharge planning across acute care settings.
• Proven ability to collaborate with interdisciplinary teams.

Certifications/Licensure

• Current California RN license (must be attached to the resume).
• BLS certification from the American Heart Association (must be attached to the submission packet).
