Part-Time Nurse Practitioner: Annual Wellness Visits Location: Multiple Locations (Northern, Southern or Western suburbs This is apart-time, multi-site clinical position tailored for a Nurse Practitioner who excels in geriatric assessment and preventative care. In this role, you will lead Annual Wellness Visits (AWVs) in your region, ensuring our geriatric population receives comprehensive, high-quality screenings and personalized prevention plans. You will: Conduct comprehensive Annual Wellness Visits at various Assisted Living, Independent Living, and Long-term Care facilities. Perform detailed cognitive screenings, functional assessments, and fall risk evaluations. Lead medication reconciliation and facilitate meaningful Goals-of-Care discussions. Develop and update patient-centered Care Plans based on AWV findings to address chronic conditions and preventative health goals. Identify and enroll eligible patients into Chronic Care Management (CCM) programs, ensuring they receive the continuous support necessary for complex health needs. Ensure thorough documentation and clinical coding to capture all wellness measures.
