Care Pavilion Nursing and Rehab , a premier subacute care center located in the heart of Philadelphia, Pennsylvania, is seeking a compassionate and dedicated Social Worker to join our team.

Job Summary

We are seeking a motivated Social Worker to provide high-quality social work services to our residents and their families. The successful candidate will possess excellent communication and interpersonal skills, with the ability to work effectively with a diverse population. As a member of our interdisciplinary team, you will be responsible for assessing social service needs, developing and implementing individualized plans of care, and facilitating connections with community resources.

Responsibilities

• Conduct initial and ongoing assessments of each resident's social service needs

• Develop and implement individualized plans of care in collaboration with the interdisciplinary team

• Coordinate connections with community resources, including home healthcare agencies, hospices, and other social services

• Provide support and counseling to residents and their families, addressing concerns and needs related to social services

• Maintain accurate and up-to-date records of resident social service needs and interventions

• Participate in staff meetings and contribute to the development of policies and procedures related to social services

Requirements

• Bachelor's degree in Social Work or related field

• Licensure as a Social Worker in the State of Pennsylvania

• 2 years of experience in a healthcare setting

• FULL_TIME position with competitive pay (salary to be determined)
