Responsibilities Evaluate physician medication orders for appropriateness and potential interactions; recommend therapy adjustments Maintain patient database with medical history, lab data, and medication records Order and evaluate lab tests for high-risk conditions Collaborate with healthcare team members Document all communications in medical records Ensure safe procurement, storage, and disposal of medications Maintain documentation in compliance with regulations Report medication errors to the Pharmacy Manager Assist staff with medication procedures Perform other duties as assigned Qualifications Bachelor’s Degree in Pharmacy; Pharm D preferred One year experience in long-term care Licenses/Certifications as required by regulations Attention to detail, goal-oriented, team player, good communication skills Join PharMerica, part of BrightSpring Health Services, a leader in long-term care pharmacy services dedicated to employee growth and community health.
