Key Responsibilities

• Provide physical therapy assistant services to patients in a home health setting
• Manage a productivity caseload of approximately 30–34 points per week
• Document patient care and progress using Home Care Home Base (HCHB) electronic documentation system
• Travel to patient homes within the northern region of the service area

Required Qualifications

• Active PTA license in the state of Connecticut
• Prior home health experience required
• Proficiency with Home Care Home Base (HCHB) documentation platform required
• Must reside within approximately 40 miles of the Mansfield, CT office area to be eligible

Preferred Experience / Skills

• Experience managing productivity-based caseloads in a home health environment
• Strong time management and independent work skills suited for field-based care
