NURSE PRACTITIONER

Signify Health seeks an In-Home Nurse Practitioner or Physician Assistant to perform comprehensive health evaluations in patients’ homes. You’ll assess complex, often vulnerable members, review histories and medications, and identify care gaps using Signify’s advanced technology and analytics.
