Social Services Director Stroudsburg Post Acute Nursing and Rehabilitation We are seeking a dedicated and experienced Social Services Director to join our team at Stroudsburg Post Acute Nursing and Rehabilitation . As a Full-time social worker, you will play a vital role in delivering exceptional care to our residents. Responsibilities Develop preliminary and comprehensive assessments for each resident, identifying their social problems and needs. Collaborate with residents and their families to develop and review individualized plans of care, ensuring their voices are heard and valued. Coordinate discharge planning to ensure a seamless transition for our residents back into their communities. Create written plans of care that outline goals and objectives for addressing each resident's social problems and needs. Ensure timely and efficient completion of social services components in Minimum Data Set (MDS) assessments. Review nurse's notes to monitor the effectiveness of care plans and make necessary adjustments. Provide exceptional customer service to our residents, families, and staff. Additional Requirements Bachelor's degree in Social Work, Sociology, or a related field (or Master's degree preferred). Minimum 2 years of experience in social work, preferably in a healthcare setting. Strong communication, assessment, and planning skills. Ability to work effectively in a fast-paced environment and prioritize multiple tasks. Compassion, empathy, and a commitment to delivering person-centered care.
