Job Title: Registered Nurse Profession: Nursing Specialty: Home Health Duration: 13 weeks Shift: Day Hours per Shift: 8 hours (08:00 - 17:00) Experience: Minimum 1 full year of Adult Home Health or Hospice experience License: Must have license in hand Certifications: CPR, BLS Must-Have: - Proficiency in HomeCare HomeBase and OASIS used within the last 6 months - Reliable transportation with current driver's license and agency-required liability insurance Description: Provides, coordinates, and directs the provision of home nursing care according to physician's orders. Care is based on agency policies and procedures through the competent application of the nursing process. Responsible to the Clinical Manager. Directly supervises and evaluates Home Health Aides and Licensed Practical Nurses/Licensed Vocational Nurses. Home visits will be required. Periodic office meetings are also part of the role. The average number of patients per day is 5-8. Each type of visit and activity, including documentation and travel, holds a different point value for productivity. Contractors will be assigned between 5-10 patients per day, roughly equating to 6-8 points. If a situation arises where low patient census affects financial needs, discussing options to float to a nearby clinic is encouraged. Types of cases include Start of Care, Wound Care, Follow-up Visits, etc. Candidates must have recent specialty experience; first-time travelers will not be accepted. Weekend requirements vary by location and should be discussed during the interview. An on-call requirement may apply, varying by location and profession. Candidates must be comfortable traveling up to 75 miles from the home clinic, with mileage reimbursed at the IRS standard rate. All documentation must be completed within 24 hours of the patient appointment. Failure to comply with documentation timely may result in a cancellation of contract. There are no guaranteed hours with this contract; clinicians must meet weekly productivity points of 30.
