The Greater Brookline per diem pool is a team of nurses who deliver requested coverage to area practices. Locations include Jamaica Plain, Brookline, Newton, Chestnut Hill, Chelsea, and Wellesley. Job Summary Reporting directly to the Practice Manager and Senior Director of Nursing, the Team Nurse position is responsible for performing the duties of an RN/LPN in an outpatient primary care or family medicine practice. The Team Nurse serves as a clinical resource for the medical practice, providing high‑quality direct patient care in a team‑based model to our Beth Israel Deaconess HealthCare (BIDHC) patients. The role enhances the safety, effectiveness, and efficiency of patient care through support and delegation of clinical and administrative functions to the Medical Assistant (MA) staff. Responsibilities include educating and training the MA team and patients on general and focused health education for preventive, chronic, and episodic health care issues, and coordinating patient‑centered care, supporting transitions between levels of care, and counseling patients to improve their health status. Essential Responsibilities Coordinate and facilitate quality clinical management of a broad outpatient population across the continuum of care. Collaborate with providers to identify needs for preventive care, evidence‑based chronic disease management, and care coordination. Provide general and specific healthcare education and work directly with patients to identify and monitor self‑management goals. Collaborate with community‑based providers, follow up on ED visits and inpatient discharges, and assist providers with exams, treatments, and procedures as needed. Provide skilled telephonic triage, assessment, education, and coordination of healthcare and community resources. Perform a direct patient care role; nursing visits include Annual Wellness Visits (AWVs) and Chronic Care Management (CCM) for BIDHC patients. Manage INR and teach patients on Coumadin using a standardized protocol. Administer vaccines and medications by parenteral, oral, rectal, or nasal routes per provider orders and BIDHC Medication Administration policy guidelines. Perform a wide range of nursing activities (e.g., wound care, point‑of‑care testing) under provider direction and accurately document all aspects of patient care in the Electronic Health Record. Schedule tests and referrals for specialty care on provider order and monitor follow‑up of abnormal test and imaging results. Oversee inventory and ordering management of clinical supplies and equipment; ensure safe, accurate, and clinically appropriate collection, storage, and processing of all biologic specimens. Provide oversight, delegation, and act as a resource to unlicensed staff, including MAs, Certified Nursing Assistants, and Nursing Students; provide ongoing education and training to practice staff. Maintain compliance with mandatory annual training and update clinical knowledge and skills through consultation, literature review, and formal and informal education. Ensure PHI is used only for treatment, payment, and operations purposes; uphold patient safety practices to prevent or eliminate risk of patient harm; promptly report adverse patient events and near misses to the BIDHC Risk Management Department. Required Qualifications Graduate from an accredited Nursing Program (Bachelor’s degree in Nursing preferred). License: Registered Nurse required, or Licensed Practical Nurse required. Certificate: Basic Life Support required. 1–3 years of related work experience in a clinical setting, preferably primary care. CPR certification. Advanced skills with Microsoft applications (Outlook, Word, Excel, PowerPoint, Access) and other web‑based applications. Ability to assume responsibility for coordination of patient care in an outpatient setting while working as part of a team of clinical and non‑clinical staff. Oral Communications: Comprehend and converse in English to communicate effectively with medical center staff, patients, families, and external customers. Knowledge: Demonstrate full working knowledge of standard concepts, practices, procedures, and policies; apply them in varied situations. Team Work: Act as a team leader for small projects or work groups, creating a collaborative and respectful environment and improving workflows; results may impact operations of one or more departments. Customer Service: Provide a high level of customer service to patients, visitors, staff, and external customers in a professional, respectful manner; stay calm in stressful situations. Physical Nature of the Job Light work: Exerting up to 20 pounds of force frequently to move objects. Some elements of the job are sedentary, but the employee will be required to stand for periods of time or move throughout the hospital campus. Pay Range $32.24 – $55.32 per hour. The pay range listed is the base hourly wage range the organization reasonably and in good faith expects to pay for this position at this time. Actual compensation may exceed the base rate depending on seniority, education, training, relevant experience, certifications, geography, job responsibilities, shift differentials, call pay, premium pay, overtime pay, and other applicable factors permissible by law.
