$45-$50/hour Ultimate Therapy is seeking a Physical Therapist (PT) to join our rehabilitation team. This position provides the opportunity to work with residents in a skilled nursing and post-acute care setting, helping them improve mobility, function, and overall quality of life. Position Responsibilities:

$45-$50/hour Ultimate Therapy is seeking a Physical Therapist (PT) to join our rehabilitation team. This position provides the opportunity to work with residents in a skilled nursing and post-acute care setting, helping them improve mobility, function, and overall quality of life. Position Responsibilities:
- Conduct physical therapy evaluations and develop individualized treatment plans based on resident needs and goals
- Provide therapy services focused on mobility, gait training, balance, strengthening, endurance, transfers, and pain management
- Collaborate with PTA staff, nursing, physicians, and interdisciplinary team members to coordinate resident care
- Monitor resident progress and adjust treatment approaches as appropriate
- Educate residents and families on therapy goals, exercises, and safety strategies
- Maintain accurate and timely documentation in accordance with facility and regulatory requirements
- Established rehabilitation program serving both short-term rehabilitation and long-term care residents
- Collaborative environment with PT, OT, SLP, and nursing professionals
- Supportive clinical leadership and strong interdisciplinary communication
- Focus on resident-centered care and meaningful functional outcomes
