Job Description We are seeking a Physical Therapist to provide rehabilitation services to patients in a Skilled Nursing Facility (SNF) and Long-Term Care setting. The ideal candidate will be passionate about helping patients improve mobility, function, and overall quality of life. Key Responsibilities Evaluate and develop individualized treatment plans Provide physical therapy services to residents in a skilled nursing and long-term care environment Monitor patient progress and adjust treatment plans as needed Collaborate with physicians, nursing staff, and interdisciplinary teams Maintain accurate and timely documentation Requirements Active Physical Therapist License Experience in Skilled Nursing Facility (SNF) or Long-Term Care preferred Strong communication and patient care skills New graduates and therapists with less than 1 year of experience are encouraged to apply Assignment Details 13-Week Contract • 36 Guaranteed Hours per Week • Overtime available after 40 hours Join a dedicated rehabilitation team and make a meaningful impact on patient recovery and quality of life. #J-18808-Ljbffr
