(Flexible schedule, time built in to do documentation) Mileage Reimbursement 17 days of PTO, 12 PAID holidays, 6 sick days Responsibilities Conduct home health assessments to determine nursing equipment needs, incorporating participant preferences and goals; participate in IDT meetings to develop care plans. Deliver and document nursing interventions per care plans, including maintaining a safe environment, following physician orders, administering medications, and performing tests and treatments. Educate participants, families, and caregivers on disease processes, self-care, and prevention strategies. Coordinate and monitor home health schedules with the Home Health Manager or scheduling contact. Collaborate with the WelbeHealth Physician and IDT to ensure smooth care transitions across settings (home, hospital, SNF, etc.). Job Requirements Graduate of an Accredited School of Nursing Unencumbered RN License required Minimum of one (1) year of nursing experience in a clinical setting with a frail or elderly population, home health agency experience preferred Reliable means of transportation Benefits 401 K savings + match Medical insurance coverage (Medical, Dental, Vision) Bonus eligibility your hard work translates to more money in your pocket Salary/Wage base range for this role is $51.63 - $68.06 hourly + Bonus + Equity.
