Travel Physical Therapist (PT) – Home Health

Sarasota, Florida

We are seeking a Travel Physical Therapist for a 20-week Home Health contract in Sarasota, Florida , covering the Sarasota/Charlotte County area. This opportunity is a great fit for an experienced home health PT who enjoys independent field-based care and helping patients improve mobility, strength, safety, and functional independence in their homes.

Responsibilities

• Perform comprehensive home health PT evaluations and reassessments.
• Develop individualized treatment plans based on patient needs and functional goals.
• Provide gait, balance, transfer, strengthening, mobility, and therapeutic exercise interventions.
• Assess patients for walkers, wheelchairs, transfer equipment, and other assistive devices.
• Treat orthopedic, neurological, cardiopulmonary, post-surgical, and medically complex patients.
• Educate patients and caregivers on home exercise programs, medications, safety, and injury prevention.
• Complete timely home health documentation and coordinate care with the interdisciplinary team.
• Identify and report changes in patient status and signs of potential abuse or safety concerns.

Requirements

• Florida PT license required.
• BLS required.
• Home Health experience required.
• Minimum 1 year of experience required.
• OASIS training preferred.
• Valid driver’s license and reliable vehicle required for home visits.
• Strong documentation and independent case-management skills required.
