Shift Type: Monday-Friday, 8:00 AM - 5:00 PM (flexible based on patient availability)

Hours per Week: 40

Candidate Pay Rate: $70 per hour

Roles & Responsibilities

• Restore function and minimize disability for patients following disease or injury
• Help clients achieve their maximum performance and function
• Evaluate and treat patients using medically prescribed physical therapy programs
• Supervise and guide Physical Therapy Assistants to ensure quality care
• Conduct initial admission and evaluation visits to assess patient needs
• Collaborate with the Clinical Team Manager by reporting OASIS scores in a timely manner
• Document clinical findings, interventions, goals, and discharge plans to ensure continuity of care

Qualifications

• One year of Home Health experience required
• Must hold an active Physical Therapy license in California
• BLS/BCLS certification required
• Ability to work independently and manage a caseload
• Strong communication and documentation skills

Knowledge & Skills

• Knowledge of home health regulations and OASIS documentation
• Skilled in physical therapy evaluation and treatment techniques
• Ability to supervise support staff
• Excellent time management and organizational skills
• Compassionate and patient-centered approach
