Position Overview

As a Licensed Practical Nurse (LPN) , you will provide direct resident care , monitor health and vital signs, maintain accurate documentation, and work closely with Registered Nurses (RNs) and Unit Managers to ensure residents receive individualized, high-quality care. You will also help foster a positive, professional environment for the entire care team.

Key Responsibilities

• Deliver direct personal care to residents and monitor overall health and well-being

• Observe, assess, and record resident vital signs and changes in condition

• Collaborate with RNs and Unit Managers to implement and maintain care plans

• Maintain accurate and timely documentation in compliance with company and state regulations

• Provide support, guidance, and training to residents, families, and team members as needed

• Promote a safe, professional, and compassionate environment for residents and staff

Qualifications

• Current Licensed Practical Nurse (LPN) license in Massachusetts

• Long-term care experience preferred but not required

• Strong communication, teamwork, and organizational skills

• Compassionate, dependable, and dedicated to resident-centered care

• Ability to work flexible schedules across all shifts

Shifts & Positions

• All shifts available : 7am–3pm, 3pm–11pm, 11pm–7am

• Full-Time, Part-Time, and Per Diem opportunities
