• Board Certified Urology or Truly Board Eligible (0-5 years within resideny completion) - (REQUIRED)
• Active CO license - (HIGHLY PREFERRED) - Candidates must obtain during credentialing and prior to start.
• BLS and ACLS - (HIGHLY PREFERRED) - Candidates must obtain during credentialing and prior to start
• Most recent NPDB Self Query Report (within 30 days active) - (REQUIRED) * no exemption, required at presentation
• Does the provider require visa support - J1 or H1B? (note in "Additional Submission Details" field) - (REQUIRED)
• DEA (must be obtained prior to start) - (note in "Additional Submission Details" field) - ( REQUIRED ) * no exemptions
• DEA Controlled Substance License (required once hired)
