In-Home Nurse Practitioner or Physician Assistant Full Time - Erie

About the Role Signify Health, part of CVS Health®, is seeking a full‑time Clinician (Nurse Practitioner or Physician Assistant) to provide in‑home health evaluations. You’ll play a critical role in building trusted relationships and delivering comprehensive, personalized health evaluations right to people’s front doors. Job Highlights Each visit lasts approximately 45 minutes and includes a medication and medical history review, a physical assessment, and, when ordered by the person’s health plan, additional diagnostic tests such as diabetic eye exams or spirometry. You will use an iPad and an intuitive clinical workflow for each evaluation. You are not required to prescribe medicine or order lab tests. Responsibilities Bring your heart into every visit, joining a national network of purpose‑driven clinicians dedicated to improving health outcomes. Work with Signify Health as a clinician colleague, fulfilling the requirements of the specified role. Be flexible to travel locally and within licensed states, as needed. Conduct virtual visits as required. Provide all Signify Health diagnostic and preventive services, including spirometry, diabetic eye exam, bone density screening, and other products. Work Environment Your primary workplace is the private residence of health‑plan members. You must be able to safely perform duties in homes that may contain pets, secondhand smoke, and other potential airborne allergens or irritants. Qualifications Active, unrestricted license(s) in the coverage area(s). Board certification (required). Multi‑state licenses (preferred). Able to cover the following primary locations: Anticipated weekly hours: 40 Hours & Compensation Time type: full time. Pay range: $87,035.00 – $187,460.00 (base hourly rate or annual salary). The actual base salary offer depends on experience, education, geography, and other factors.
