*What You'll Do*

You'll provide skilled nursing care in the place patients feel most comfortable their own homes. Your responsibilities include:

* Delivering care per physician orders, agency policy, and regulatory guidelines
* Performing OASIS assessments and documentation from admission through discharge
* Updating and re-evaluating plans of care at least every 60 days
* Serving as case manager, coordinating multidisciplinary care across nursing, therapy, home health aides, and social services
* Maintaining accurate, timely clinical documentation
* Participating in Quality Assurance and Performance Improvement (QAPI) programs
* Ensuring all care meets compliance and regulatory standards

*Who We're Looking For*

* Active NYS Registered Nurse license (required)
* Valid NY Driver's License, CPR/BLS certification, and COVID-19 vaccination (as applicable)
* 1+ year of RN experience preferred new graduates are welcome and will receive full onboarding support
* Home care, acute care, or rehab background is a plus
* OASIS experience helpful but not required
