Responsibilities Provide direct patient care to residents in a variety of settings, including skilled nursing facilities, rehabilitation programs, and post-acute care settings Administer medications, treatments, and procedures as ordered by physicians and as part of a collaborative care team Monitor patient conditions, report changes, and document patient information accurately and thoroughly Collaborate with physicians, nurses, and other healthcare professionals to develop and implement individualized patient care plans Educate patients, families, and caregivers on self-care techniques, disease prevention, and health promotion Participate in quality improvement initiatives and contribute to the development of policies and procedures Uphold Valley Manor's commitment to excellence in patient care and service Requirements Current licensure as a Licensed Practical Nurse (LPN) in the state of Pennsylvania High school diploma or equivalent required; associate's degree or diploma preferred Ability to work in a fast-paced environment and prioritize tasks effectively Strong communication and interpersonal skills, with the ability to work collaboratively with colleagues and families Familiarity with electronic medical records and basic computer skills About the Role Join a team of dedicated healthcare professionals who are passionate about delivering exceptional patient care and service.
