Inpatient Case Manager: Discharge & Care Coordination

PAM Health Rehabilitation Hospital of Beaumont is seeking a Case Manager to coordinate care for designated patient populations. This role involves reviewing admission records for proper resource utilization and assessing discharge needs to develop appropriate plans. The ideal candidate will have 3 to 5 years of experience in inpatient settings. A relevant state licensure and current BLS certification are required.
