Assistant Director of Nursing

The Assistant Director of Nursing (ADON) is a key member of the clinical leadership team and works closely with the Director of Nursing to oversee the delivery of safe, high-quality, compliant, and patient-centered care. The ADON assists with the day-to-day management and oversight of clinical operations, including skilled nursing and private duty nursing services, clinical supervision of field staff, patient assessments and reassessments, Plans of Care, physician orders, staff education and competency, quality assurance, incident investigation, clinical triage, and coordination of care. The ADON serves as a clinical resource to patients, families, nurses, caregivers, office staff, physicians, facilities, and other healthcare partners. This position requires strong clinical judgment, leadership, organization, communication, and the ability to effectively manage multiple priorities in a fast-paced home-care environment. Clinical Oversight & Patient Care Conduct initial nursing assessments, admissions, reassessments, supervisory visits, and other clinical visits as required. Perform new assessments for long-term care/insurance and other applicable patients. Complete post-hospitalization, post-fall, change-of-condition, and other clinically indicated assessments and follow-up visits. Conduct supervisory visits for skilled nursing, private duty nursing, home health aide, and other applicable services. Evaluate patient condition, care needs, home environment, caregiver capabilities, and appropriateness of services. Identify changes in condition and ensure appropriate intervention, physician notification, documentation, and follow-up. Provide clinical triage and guidance to field staff, patients, families, and office personnel. Assist in resolving clinical concerns and complex patient-care situations. Ensure continuity and coordination of care following hospitalization, emergency department visits, changes in condition, or other significant events. Plans of Care, Orders & Documentation Develop, review, revise, and update Plans of Care in accordance with patient needs, physician orders, payer requirements, and applicable regulations. Obtain new and updated physician orders for skilled patients as needed. Ensure clinical documentation accurately reflects the patient's condition, services provided, interventions, and communication with physicians and other members of the care team. Review clinical records for completeness, accuracy, timeliness, and compliance. Follow up on missing, incomplete, or inconsistent clinical documentation. Assist with maintaining hospital/discharge tracking and ensuring appropriate clinical follow-up. Monitor and follow up on clinical inquiries and outstanding clinical items to ensure timely resolution. Clinical Staff Supervision & Development Assist the DON with clinical supervision and oversight of nurses, home health aides, and other clinical field staff. Conduct nursing orientation and participate in orientation of home health aides and other applicable staff. Provide ongoing clinical education, training, coaching, and support to field staff. Identify educational opportunities based on patient needs, incidents, documentation concerns, performance issues, regulatory requirements, and trends. Evaluate staff clinical performance through supervisory visits, observation, documentation review, competency validation, and other appropriate methods. Address clinical performance concerns promptly and collaborate with the DON and Human Resources when coaching, corrective action, remediation, or additional training is indicated. Serve as an accessible clinical resource for field employees. Promote accountability, professionalism, communication, patient advocacy, and adherence to standards of care. Quality Assurance, Compliance & Risk Management Assist the DON in monitoring clinical operations for compliance with company policies, professional standards, payer requirements, accreditation standards, and applicable federal and Florida regulations. Participate in clinical quality assurance and performance improvement activities. Review incidents, complaints, clinical concerns, hospitalizations, falls, medication issues, and other adverse or potentially adverse events. Conduct and/or participate in investigations involving patient care, clinical staff, complaints, incidents, or potential policy violations. Obtain statements, review documentation, interview involved parties, identify contributing factors, and assist with corrective action and follow-up. Identify patterns, trends, or areas of clinical risk and bring concerns to the attention of the DON. Assist in implementing corrective actions, education, process improvements, and other measures designed to improve patient safety and quality of care. Maintain confidentiality and ensure compliance with HIPAA and other privacy requirements. Clinical Coordination & Operations Collaborate closely with scheduling and operations staff regarding patient needs, staffing concerns, caregiver/nurse compatibility, changes in condition, and other clinical considerations affecting scheduling. Answer and triage incoming clinical calls and assist scheduling staff with clinical questions and concerns. Communicate significant clinical information to appropriate members of the care team. Participate in clinical, operations, patient-care, quality, and leadership meetings as assigned. Assist with communication to clinical employees and tracking of required responses, education, documentation, or follow-up. Coordinate with physicians, hospitals, facilities, pharmacies, therapy providers, insurance representatives, case managers, and other healthcare professionals as appropriate. Assist with medical supply coordination and delivery when necessary to support patient care. Participate in community outreach, marketing, referral-source relationships, or business-development activities when clinical representation is beneficial. On-Call Responsibilities Participate in the clinical on-call rotation as assigned. Provide clinical guidance and decision-making support for after-hours patient and employee concerns. Review on-call activity and ensure issues requiring additional intervention are appropriately followed up. Escalate significant clinical events or concerns to the DON or other appropriate leadership personnel. Ensure appropriate documentation and communication of after-hours clinical events. Leadership Responsibilities The ADON is expected to function as a member of the leadership team and demonstrate sound judgment, professionalism, accountability, and discretion. The ADON will support the DON in maintaining clinical standards throughout the organization and may be delegated responsibility for specific clinical programs, patients, staff members, quality initiatives, audits, investigations, or operational processes. The ADON is expected to identify problems proactively rather than solely responding to them and to recommend solutions that improve patient care, employee performance, regulatory compliance, efficiency, and communication. When delegated by the DON, the ADON may provide clinical direction and oversight to clinical personnel and assist in managing clinical operations during the DON's absence. Patient & Family Relations Establish and maintain professional, compassionate relationships with patients and families. Respond promptly and professionally to patient and family questions or concerns. Assist with resolving complaints and service concerns involving clinical care. Educate patients and families regarding Plans of Care, services, clinical expectations, and changes in care. Advocate for patient safety, dignity, preferences, and continuity of care. Maintain appropriate professional boundaries while providing responsive and compassionate service. Qualifications Current, unrestricted Registered Nurse (RN) license in the State of Florida. Nursing experience appropriate to the patient populations and services provided by the organization. Home health, private duty nursing, pediatric, geriatric, or clinical leadership experience strongly preferred. Supervisory or nursing leadership experience preferred. Physical & Work Requirements The ADON must be able to perform the essential functions associated with clinical nursing and home-care operations, including traveling to patient homes and other locations throughout the service area, performing patient assessments, using standard nursing equipment, communicating effectively with patients and staff, and responding to clinical needs in a variety of home and community environments. The position includes a combination of office-based responsibilities, field visits, telephone/remote clinical support, meetings, and participation in an on-call rotation.
