Physical Therapist
$80 - $100 per hourCONSUMER DIRECTED SERVICES INC.
Role In-Home Physical Therapist (PT) providing patient-centered care in patients' homes. Key Responsibilities Conduct initial evaluations and develop personalized treatment plans. Administer interventions to improve strength, balance, mobility, and independence. Educate patients and caregivers on exercises, safety, and home modifications. Monitor progress and adjust treatment plans accordingly. Accurately document evaluations, treatments, and progress notes. Collaborate with healthcare team members to ensure seamless care. Provide guidance and support to encourage active patient participation. Maintain equipment, supplies, and records for home therapy sessions. Qualifications Licensed Physical Therapist in Massachusetts. Preferred experience in home health or outpatient therapy. Strong assessment, communication, and problem-solving skills. Ability to work independently with a flexible schedule. Reliable transportation and valid driver's license. Compensation $80.00 - $100.00 per hour. #J-18808-Ljbffr CONSUMER DIRECTED SERVICES INC.
